You can customize the forms in documents.
1. Navigate to Documents > Custom Form Templates.
The Custom Form Templates page is displayed.
2. Click Add Custom Form.
The Custom Form Template dialog is displayed.
3. Enter the name of the form in the Form Name field.
4. Select the scope of the form from the Scope drop-down list.
5. Enter any note if needed in the Notes field.
6. Click Add.
The new custom form is added.
7. Add the required form components by left-clicking the components on the right-hand side of the page. The form components that are available are as follows.
Note: On mouse over on the components, icons are displayed to edit (), copy (), or remove ()the form component.
Header - This component allows you to add a section header.
i. Click icon.
The Header dialog is displayed.
ii. Enter the name for the header in the Label field.
iii. Select the header type from the Type dropdown list as needed. The available header types are -
h1 - used to indicate the main heading
h2 - used to indicate the subheading of h1
h3 - used to indicate the subsection of h2 heading
h4 - used to indicate the subsection of h3 heading
h5 - used to indicate the subsection of h4 heading
h6 - used to indicate the subsection of h5 heading
iv. Click Close.
The header is added to the form.
Text field - This component allows you to add a text field.
i. Click icon.
The Text field dialog is displayed.
ii. Enter the name for the text field in the Label field.
iii. Enter an informative description of the text field in the Help Text field.
iv. Enter the filler text in the Placeholder field.
v. Enter the value of the text field in the Value field.
vi. Select the text type from the Type field.
vii. Select the maximum length of the text from the Max Length dropdown list.
viii. Click Close.
The text field is added to the form.
Text Area - This component allows you to add text that is lengthier, example: notes or description.
i. Click icon.
The Text field dialog is displayed.
ii. Enter the name for the text area in the Label field.
iii. Enter an informative description of the text area in the Help Text field.
iv. Enter the filler text in the Placeholder field.
v. Enter the value of the text area in the Value field.
vi. Select the text type from the Type field.
vii. Select the maximum length of the text from the Max Length dropdown list.
viii. Select the number of rows the text area can have from the Row dropdown list.
ix. Click Close.
The text area is added to the form.
Note: You can resize the text area by holding the left mouse button down on and resizing the text area as needed.
Paragraph - This component allows you to enter the text for instructions or guidance for the users.
i. Click icon.
The Paragraph dialog is displayed.
ii. Enter the text needed to be displayed as paragraphs in the Content textbox.
iii. Select the type of paragraph from the Type field. The available types of paragraph fields are -
p - used to enter paragraphs such as descriptions or notes
address - used to enter the address of the user or customer or location
blockquote - used to indicate the quotation of a large section of text from another source
canvas - used to draw graphics
output - used to represent the result of a calculation
iv. Click Close.
The paragraph is added to the form.
Select - This component allows you to add dropdown lists for the form.
i. Click icon.
The Select dialog is displayed.
ii. Enter the name for the selection list in the Label field.
iii. Enter an informative description of the text area in the Help Text field.
iv. Enter the filler text in the Placeholder field.
v. Check the box Allow Multiple Selections if you want to enable many options.
vi. Enter the labels for the required options in the Options field.
vii. Click Add Option + if you wish to add more options.
viii. Click Close.
The selection list is added to the form.
Checkbox Group - This component allows you to add a checkbox or a group of checkboxes for the form.
i. Click icon.
The Checkbox Group dialog is displayed.
ii. Enter the name of the checkbox group in the Label field.
iii. Enter an informative description of the text area in the Help Text field.
iv. Check the box Toggle if you wish to allow the users to have a switch option.
v. Check the box Inline if you wish to display the checkboxes in a line.
vi. Check the box Enable “Other” if you wish to allow users to enter an unlisted option.
vii. Enter the labels for the required options in the Options field.
viii. Click Add Option + if you wish to add more options.
ix. Click Close.
The checkbox group is added to the form.
Radio Group - This component allows you to add a radio button or radio button selection group.
i. Click icon.
The Radio Group dialog is displayed.
ii. Enter the name of the Radio group in the Label field.
iii. Enter an informative description of the text area in the Help Text field.
iv. Check the box Inline if you wish to display the radio button in a line.
vi. Check the box Enable “Other” if you wish to allow users to enter an unlisted option.
vii. Enter the labels for the required options in the Options field.
viii. Click Add Option + if you wish to add more options.
ix. Click Close.
The radio group is added to the form.
Date Field - This component allows you to add a date calendar dropdown to allow the users to select a date.
i. Click icon.
The Date Field dialog is displayed.
ii. Enter a text for selecting the date in the Label field.
iii. Enter an informative description of the text area in the Help Text field.
iv. Enter a filler text in the Placeholder field.
v. Click Close.
The date field is added to the form.
Number - This component allows you to add a number limit in your form.
i. Click icon.
The Number dialog is displayed.
ii. Enter a text for selecting the number in the Label field.
iii. Enter an informative description of the text area in the Help Text field.
iv. Enter a filler text in the Placeholder field.
v. Enter a value in the Value field.
vi. Select the minimum number as needed in the Min dropdown list.
vii. Select the maximum number as needed in the Max dropdown list.
viii. Select the number skip count from the Step dropdown list.
ix. Click Close.
The number field is displayed.
Note: The components can be rearranged in the form as needed by holding the left button down on the component and dragging and dropping them.
8. Click Save Form.
The custom form template is saved.
